    • The tax departments of the jurisdictions listed on this form have agreed that it can be used on an optional basis in those jurisdictions by a marketplace facilitator/provider to certify to its marketplace sellers that as of the stated collection date, it will collect and remit sales or use tax and other required taxes on sales facilitated through the marketplace facilitator/provider’s platform.
